By using the VSP Choice network, members will have care from great eye doctors and quality eyewear at low out-of-pocket costs.

- 0-12 months in force: $500 annual maximum coverage
- 13-24 months in force: $1,000 annual maximum coverage
- 25+ months in force: $1,500 annual maximum coverage

- Eye-care benefits administered by VSP
- Extensive network of independent optometrists and eyeware providers
- $15 copay for refractive exams
- $35 copay for frames (every 2 years), lenses (every year)
- $150 allowance for contacts per year
